To initiate the application process, simply fax a letter of intent, indicating your name, specialty, Tax Identification Number, and contact information to our Provider Relations Department at 702-248-2622. An application packet including fee schedules will be forwarded. The contracting process usually takes 3 – 4 weeks, depending on how quickly you can provide the requested credentials.
