This site will allow you to access claim status information for claims administered by Mediversal prior to January 1, 2009. Claims for terminated members are only available for six months following their termination date.

All Family CareNet Participating Provider Agreements with Mediversal, Inc. have been assigned to Indiana Health Network (IHN). IHN is a subsidiary of Connecticut General Life Insurance Company (CGLIC) following CGLIC’s purchase of IHN, and other healthcare business from Great-West Life & Annuity Insurance Company on April 1, 2008.  IHN will now administer the Family CareNet PPO. 

Effective April 1, 2009, all communication and correspondence regarding your Family CareNet Participating Provider Agreement will be handled by representatives at IHN.  Below is the contact information:
